Your charity's government registration documents, showing your name and role. For example, an IRS Form 990 or a determination letter (US), a document from the Charity Commission (UK), a Responsible Persons document (Australia), or a Directors/Trustees and Like Officials worksheet (Canada)

Your charity's bank statement, showing your name as an authorized signer

Your most recent pay stub (accepted if charity website provided)

Meeting minutes

Forms or documents you have created and/or filled out

2) Confirming your identity

We need to confirm your identity to ensure we are speaking to the correct individual. Please attach a colored picture of your government-issued photo ID in the attachment box at the bottom of this page. Please make sure you upload an image that is JPG, PNG, or PDF formatting. HEIC files are not accepted.
